It is confusing, Bob.
First, regardless of the source (CAJA or commercial farmacias) not all medicatons that are available in the U.S. are available at all here in Costa Rica. Conversely, some things that are not available in the U.S. are available here. It’s a mixed bag.
Second, of that total list of medications that are available here in Costa Rica, not all of them are provided by the CAJA. For instance, I’m on an anti-cholesterol medication. The single anti-cholesterol drug that’s available from the CAJA isn’t very effective and my endocrinologist (who does not practice within the CAJA system, by the way) recommends against it.
The same is true of one of the two diabetes oral meds I take. The expensive one isn’t available from the CAJA. The good news for some of us diabetics is that the CAJA does provide injectable insulin which is quite expensive over the counter here.
The other downside of getting meds from the CAJA is that you must have a new prescription every month. That means either visiting the clinic and maybe a long wait or paying a private practice physician to write the scripts.
On the upside, however, is that if you need them the CAJA will also dispense vitamins and minerals.

Well of course, if you are vitamin D deficient then by all means stand out in the sun for a few hours a day….:D.
