Yes, that should include everything.
Read the online newspapers, check out various forums.
All of the places you mention have many ex-pats so rent will be higher. As maravilla mentioned, if you expect to rent furnished accommodations, the cost will rise accordingly.
It will depend on what you want or like, on where you should look.
If you have never visited Costa Rica, you should really make an extended trip before making any big decisions. Living in Orosi or San Ramon is very different from living in Atenas.

Anything you read about the cost of living will be very, very unreliable. First, costs change daily. Second, your own lifestyle, location, wants, needs and expectations play a very large part in actual costs.

Have you actually been to Costa Rica recently? If not, an exploratory trip is essential. On that outing you can explore housing opportunities, shop in the stores, consult an attorney about the residency process, etc. Only then will you have any meaningful sense of what it’ll cost you.

An attorney’s fee of $1,500US for processing a pensionado’s residency application would be very reasonable. That would NOT include the government’s fees, the non-refundable “good conduct” deposit, the costs of obtaining and processing your documents in the U.S., the cost of issuing your cedula (ID card), pictures, etc.

Atenas, Grecia, San Ramon and all the other Central Valley “towns” each have their own personalities, climates, etc. None is, by definition, any better than the others, but they are different. Again, exploration on your own is the key.

You can, indeed, buy a one-way ticket to Costa Rica from the U.S. and a bus ticket to Nicaragua or Panama to satisfy the “exit” requirement. But again, the one-way ticket may be more expensive, and you’d have to have the bus ticket in hand before the airline will permit you to board. How ya gonna do that?
